When creating your form, you have two options. You can assign a form to a channel or a keyword.

If assigning to a channel, anytime someone uses the command /form in that channel they will see your form. You can only have one form assigned per channel.

If you assign a keyword to your form, then when someone uses the command /form keyword anywhere in Slack, they can submit your form.
