A datepicker input that allows the responder to select a date from a calendar. 

Options:

• Title: The title for the form element
• Initial date option: If you would like the input to preload with no date, the current date for the responder, or a custom date
• Initial date: The initial date that the input should be set to
• Optional: If enabled, the responder does not have to complete this input
