Checkbox Input

A checkbox input that allows the responder to tick value(s) from a list of items that the form builder provides during creation.
The items for the checkboxes are added as comma separated values. For example, to create a group of checkboxes that contains the options:
- Blue
- Red
- Yellow
During form creation, enter your options as:
Blue, Red, Yellow
Checkbox inputs can not be used with Zapier at this time.
Options:
- Title: The title for the form element
