Today I wanted to quickly discuss a few examples where the Forms for Slack app can improve the way that your team communicates through Slack.
These are ideas that I've used personally, or discussed with other teams and were all ideas that we kept in mind while building the first versions of the Slack Forms app.
Customer Feedback
Sharing positive feedback in Slack is probably something every team does. It's a great boost to morale and helps team members share some of the wins they have.
But often this feedback comes in anecdotal stories. This is a great way to share wins, but it can get lost in the chatter and means that you could be missing out on some of the key details that made that interaction a positive one.
Form Setup
- Create a form with the keyword feedback. This allows people on the team to submit feedback to any channel by just sending /form feedback.
- Assign it a field with a multi-line input so they can provide some description.
- Give it a date input so you can monitor it over time.
- Add a user input so they can say who the feedback was relevant to.
Now you have a form that can be used anywhere and can be easily tracked by just searching for the form's title.
I.T Helpdesk
Most teams will have some form of internal I.T department. For smaller teams maybe it's just one person, or if you're a larger team it could be a team of many.
But in any case, to make sure your issue is resolved the I.T team likely need specific information and context about your problem.
Giving them the specific information they need right away means less time spent waiting for them to respond, less time going back and forth, and a faster resolution for I.T issues.
Let's assume that your team has a dedicated IT support channel called #it-support that can handle these requests.
Form Setup
- Create a form and assign it to the #it-support channel. This means that any time someone types /form in the #it-support channel, they will see the form you create.
- Add a date input so people can say when the issue first started happening.
- Add a single line input to describe the type of request
- Add a multiline input so you can describe the issue in more detail
- Add a single line input so the user can add their best contact details
Bonus: You could also create another form for a #it-request channel which is just designed for requesting new IT equipment or replacements.

Design Requests
If your team is using Slack Forms, or you think there's a way you can improve how you team is chatting through Slack, I'd love to hear about it.
Anonymous Surveys
Want to just get feedback that's anonymous? All form submissions by default are anonymous and won't show the submitter's name.
You could create a form for a specific event and ask people to submit feedback via this form
